Become a Diversion Case Manager in our Clinical and Prevention Services Department and help empower youth aged 16-24. This full-time, onsite contract position is dedicated to preventing homelessness through innovative solutions.
The ideal candidate is passionate about serving at-risk youth, possessing a minimum of three years’ relevant experience. You will engage with young people to identify safe housing options and partner with families and community resources. Additionally, you will implement follow-up strategies to ensure housing stability and effectively manage housing crises.
Key Responsibilities:
• Provide support to youth at risk of homelessness
• Liaise with landlords to promote housing retention
• Deliver follow-up case management services
• Facilitate access to emergency funding
• Conduct workshops and group activities as needed

• BSW required; MSW preferred
