As part of Case Management at Unity Health Centre, the Senior Assessor Coordinator works with clients and families to ensure effective continuity of care. This position allows you to make a significant impact on the quality of life for clients in a community health context. Preference will be given to current HSAS members.

Responsibilities

• Create individualized care plans for patients
• Monitor clients’ progress and adapt plans accordingly
• Liaise with families to address their support needs
