Position Overview
In consultation with the interdisciplinary patient care team, the Occupational Therapist plans and provides occupational therapy input for residents in Long‑Term Care, participants in the PRIME program, patients in the Acute Care Program, and Out‑Patient Programs, including Easy Street and MHC for Lungs. The Therapist collaborates with clients through assessment, intervention, follow‑up, and planning to meet occupational performance goals and support healthy lifestyles, prevent illness and disability, and promote health.
Responsibilities
- Assess participants and residents to identify occupational needs and set attainable goals.
- Design and implement individualized interventions and therapeutic activities.
- Conduct follow‑up reviews to evaluate progress and adjust care plans.
- Provide quality client care through continuing professional development, research, and quality improvement initiatives.
