Social Worker (Professional Practice)

Overview

This position is responsible for providing social work services to hospital patients and families and providing excellent patient care within an interdisciplinary team setting within a variety of program areas including but not limited to critical care, respirology, medicine, stroke, surgery, family birthing centre & the emergency department. The Social Worker will provide care to clients and families in the hospital and hospital affiliates as required. Responsibilities include providing comprehensive assessment and treatment planning, educational and supportive counseling, resource identification, referral and linking, and collaborating and consulting with inter‑professional team and community based service providers. Primary area of assignment will be Maternal Newborn Care & Paediatrics with cross coverage of Emergency Department as needed.
