The Frequent Visitor Program supporting Emergency Departments across the Calgary Corridor is currently searching for a Social Worker (SW) III to join our team. This position will provide SW input and guidance for an interdisciplinary team to improve care for patients who present frequently to the Emergency Department (ED) with complex medical, mental health and social needs. This role will champion coordinated, patient-centered approaches that reduce repeat presentations, improve outcomes, and strengthen integration between acute, community, and primary care sectors.
