The Social Worker II Assisted Living Alberta plays an essential role in supporting patient transitions and coordinating care across Acute Care, Home and Community Care, and Continuing Care Home settings. This position delivers comprehensive clinical social work services with a strong emphasis on financial assessment, resource navigation, psychosocial evaluation, counseling, case management, and discharge planning. In this role, you will assist patients and families in accessing financial benefits, completing funding applications, and liaising with government programs, community agencies, and other service partners to ensure appropriate supports are in place.
