Assisted Living Alberta is dedicated to Enhancing Care in the Community, helping people be healthy, well and independent in their homes and communities. Edmonton Zone Continuing Care provides services through five main program areas: Home Living, Supportive Living, Facility Living, Transition Services and Palliative Care. We are looking for a Social Worker II to join our Home Living team to provide professional support services in the homes of clients of all ages and stages of life.

The Social Worker II works collaboratively as part of a multidisciplinary team, is involved in the case management of clients, including ongoing assessment, determination of unmet need, and care planning in consultation with clients to achieve client goals utilizing appropriate internal and external resources.
