Social Worker II - Continuing Care (Edmonton)

Assisted Living Alberta is dedicated to Enhancing Care in the Community, helping people be healthy, well and independent in their homes and communities. Edmonton Zone Continuing Care provides services through five main program areas: Home Living, Supportive Living, Facility Living, Transition Services and Palliative Care.

Responsibilities

• Provide psychosocial assessment, crisis intervention, education, advocacy, resource finding, consultation, and appropriate referrals to multidisciplinary team members.
• Deliver psychosocial assessment, resource‑based interventions, individual, group and family treatment, risk screening, counselling, case management and coordination.
• Assist with transition or discharge planning, and promote prevention, health promotion and public awareness activities.
