Join Ontario Health atHome as a Bilingual Social Worker (MSW) to provide essential care to patients in a community-focused environment. This role will enable you to work closely with patients on their journey to recovery.
Your expertise will help assess patient needs, refer them to appropriate services, and establish treatment goals in stroke rehabilitation. Each day, you will engage with diverse patient groups while contributing to a supportive team dedicated to integrated care. This full-time position runs Monday to Friday.
Key Responsibilities:
• Conduct psycho-social assessments for patients and families
• Develop and maintain detailed patient treatment plans
