RESPONSIBILITIES Conduct financial assessments and assist clients with applications for financial schemes
Collaborate with multi-disciplinary team members to ensure access to care services & financial schemes such as SMF
Perform Information & Referral, basic case management and basic counselling support functions
Conduct home visit independently or joint visit with MSWs or Home Care team where necessary
Conduct care assessment (e.g. InterRAI) ...
