The SocialPrescriber will support individuals to improve their health and wellbeing byconnecting them to non-medical community services. Working as part of amultidisciplinary team within primary care networks, they help people addresssocial, emotional, and practical needs that impact their health, such asloneliness, housing, employment, or financial challenges.
Main duties of the job
