Sacred Care Case Manager (Winnipeg)

Reporting to the Sacred Care Program Supervisor, the Sacred Care Case Manager is responsible for providing support to the Sacred Care homes. The Sacred Care Case Manager works within a team that serves to provide comprehensive case management services within a holistic circle of care.

Key Responsibilities

• Establish a positive relationship with foster parents, youth in the homes, and the support team primarily through regular home visits and ongoing communication by phone and via email.
• Conduct monthly case planning meetings in the foster homes with the assigned collaterals.
• Complete required Foster Care Licensing and service documentation.
• Maintain an accurate understanding of the services provided in each home and effectively communicate with the Program Manager and SECFS.
• Collect, review, and distribute monthly reports and incident reports for each home.
