Hours of work: Mon.– Fri. 8:30a - 4:30p flex (75 Hrs. Biweekly), some evenings and weekends might be required

• Ensuring that all Direct Care Workers facilitate a Wellness Support Plan or Individual Support Plan Process (ISP) for each client.
