Remote Case Manager - Elder Care & Care Coordination

Limitlessli is seeking a remote Case Manager to act as the primary liaison between patients and caregivers, ensuring high-quality care and effective communication. This position requires at least 3 years of case management experience and a valid Registered Nurse license (USRN NY). The ideal candidate is bilingual in Spanish and adept at managing sensitive situations with empathy.

The role involves coordinating patient care, documenting progress in HHA Exchange, and working within US Eastern Standard Time (EST). Fluency in Spanish and experience with elderly clients is preferred.
