Job Description

Position Overview: As a registered health professional (registered social worker, recreation therapist, or registered practical nurse) reporting to the Program Supervisor, you will provide tenants with short-term case management supports that help them navigate community and social services and build capacity to self-manage physical and mental health needs.

Salary: $53,244-62,220 (Registered Social Worker/Recreation Therapist)

Salary: $59,160-69,870 (Registered Practical Nurse)

Hours of Work: Full time, 40 hours per week, 8 hours a day. Schedule is based on support needs of individuals in the program, including some evenings, weekends and public holidays. As the program operates 7 days a week, an on‑call rotation of carrying a work cell phone 24/7 for one week every five weeks is required.
