Registered Practical Nurse (RPN) - Part Time - Northridge LTC (Markham)

Responsibilities

• Work as a member of the interdisciplinary team within Extendicare’s Skilled Practice Model, applying a person-centered approach to care.
• Complete and ensure residents’ clinical care assessments, care/service plans and documentation are accurate, up to date and changes are communicated as required.
• Implement and evaluate the effectiveness of care/service plans and make changes as required.
• Assist with scheduling and participate in residents’ care conferences.
• Work closely with the DOC/DHW to facilitate residents’ move in, move out and transfers.
• Provide direct resident care and medication/treatment administration and document care in compliance with regulatory and Extendicare Resident Care standards and policies and procedures.
• Process orders, safely store drugs and narcotics, and administer in compliance with all regulatory requirements, scope of practice and Extendicare policies and procedures. ...
