Overview
Registered nurses provide home-based care with a per-visit model, flexible scheduling, paid training, and travel to client homes. This role requires delivering evidence-based nursing care and coordinating with a care team.
Responsibilities
- Complete comprehensive client assessments, develop care plans, implement and evaluate care based on best evidence and the client’s needs.
- When designated as a primary provider for a client caseload, act as the point of care lead and collaborate with other care team members and support functions.
- Support the client through care transitions by collaborating and connecting the client with community support services in their local community.
Requirements
- Current registration with the College of Nurses of Ontario
- CNA specialty certification is an asset
