Located in the historical community of Fort Macleod and in close proximity to the Rocky Mountains, Fort Macleod Home Care has an opportunity for a dynamic and enthusiastic individual to join our team. Reporting to the Seniors Health- Integrated Home Care Area Manager, the Registered Nurse Case Manager is responsible for collaborating and organizing comprehensive health care services for clients in a variety of settings. The RN Case Manager has experience in the role of advocate and can balance the needs of the individual, family, and community within organizational and legislative parameters. The Fort Macleod Home Care Team is knowledgeable with a strong commitment to quality patient care. The RN Case Manager has the ability to develop creative approaches to client care and demonstrates sound critical thinking and ethical decision making. The RN Case Manager demonstrates a patient and family centered approach with all client interactions.
