Registered Nurse CONTRACT 9 month FT Contract

Overview of the Role

Provides outstanding primary care nursing services for marginalized clients, who may have severe and persistent mental illness, across the lifespan. This position is community-based.

Responsibilities

• Participates to full scope as a member of a clinical team providing nursing services for all clients
• Functions within the Mission United hub model in all daily encounters
• Completes triage assessments
• Performs comprehensive mental health assessments
• Completes assessments that are holistic and include relevant medical, psychiatric and social domains
• Prepares individualized nursing care plans that reflect the client’s goals
• Provides information and resources regarding prevention, healthy lifestyle, sexually transmitted diseases, diabetes, nutrition, alternative lifestyles, chronic diseases and terminal illnesses as appropriate
