Job Overview

Yee Hong Centre for Geriatric Care seeks a Program Coordinator to support planning, evaluation, and delivery of client programs that enhance independence and quality of life.

Responsibilities

• Design, plan, coordinate, implement and evaluate programs to enable clients to cope with illnesses and regain confidence, self‑worth and independence.
• Coach, develop, and promote an engaged, dynamic team.
• Contribute to the evaluation of staff performance.
• Work closely with client, worker, physician and caregiver to assess eligibility for service and ensure appropriate care plan design and ongoing monitoring.
• Ensure provision of personal care and administration of medication in accordance with legislation.
• Assist in development of program budget and implement effective expenditure control system.
