Position Overview

The Primary Care Manager implements strategic and operational objectives across medical homes, managing multidisciplinary teams, developing relationships with member physicians, and leading medical home optimization efforts. Reporting to the Senior Manager – Central Clinic, the role integrates PCN programs and processes.
