Responsibilities
- Lead the implementation and deployment of value-based care models including population health management and risk-based contracting
- Collaborate with providers and practice staff to optimize clinical and administrative workflows
- Implement and optimize practice use of technology solutions including EHRs and care management tools
- Develop and implement population health management programs for high-risk patient populations
- Partner with practices to execute strategies to achieve quality performance goals
- Facilitate care coordination among healthcare providers, patients, and families
- Develop performance measurement systems to evaluate VBC interventions
- Build strong, trust-based relationships with practice administrators and providers
