About the Role
You are a passionate and experienced Registered Nurse who is committed to improving the physical health outcomes of consumers experiencing mental health conditions. You will provide specialist physical health expertise across community mental health teams, supporting an integrated, holistic model of care. In this role, you will work collaboratively across both Hawthorn and Clarendon Community Mental Health Services , partnering with multidisciplinary teams to enhance the identification, monitoring, and management of physical health needs for consumers with complex and enduring mental illness.
Conduct comprehensive physical health assessments for consumers, identifying unmet health needs and contributing to holistic care planning.
