This position is responsible for medication therapy review and monitoring and overseeing the medication management process within patient care units/areas. This Pharmacist role works out of the Queen Elizabeth Hospital supporting Provincial Pharmacy Services - Ambulatory Care.
- reviews and processes medications orders in a timely manner;
- provides direct patient care activities and supports the physicians and nurses at the front line;
- develops patient care plans and contributes to overall medication management;
- supervises and supports the Pharmacy Technicians in their daily activities;
- provides resource material and information to physicians, nurses, patients and other members of the health care team regarding the safe and effective use of medications;
