Job summary
This is a dynamic and autonomous community-based role at the forefront of palliative and end of life care across Northwest Surrey. You will work in close partnership with the Nurse Consultant, contributing to day-to-day clinical leadership and service development, while supporting and developing colleagues within a collaborative, multidisciplinary environment.
As an expert practitioner, you will undertake holistic assessments, provide specialist advice, and deliver high-quality, evidence-based care that improves the experience of patients and their families at every stage of the palliative journey. You will also play a key role in education and workforce development, building capability across community nursing teams and partner organisations.
