Occupational Therapist, Temporary Full Time, Transitions Home Program, RTC (Kingston)

This position with the Providence Care – Transitions Home Program will be engaged in community work and involves commuting across the southeastern Ontario region. The Transitions Home Program works to achieve timely access to care and clients’ functional outcomes following discharge from PCH/PTCC. Therapeutic interventions will be carried out in the client’s home and community.

As the Occupational Therapist, reporting to the Clinical Program Coordinator, you will serve as the primary resource to program staff, clients, caregivers and community partners. In collaboration with program staff, you will provide assessment of assigned clients and develop and implement individual occupational therapy treatment programs.
