Job Summary

The Possibilities by Baycrest is Baycrest’s unique evidence-based memory care model. The successful candidate will support the Possibilities by Baycrest model through embodying the philosophy of care and principles, while bringing the key commitments under the model to life. Reporting to the Clinical Manager, the Registered Practical Nurse will execute individualized care plans and Possibilities Plans for the residents living in the new Terraces Memory Residence. The RPN works with each resident and family to achieve personal, psychological, physical and recreational goals within the Possibilities by Baycrest framework.

The ideal candidate will be well rounded with residential, geriatric, customer service and clinical care experiences working with older adults with dementia and responsive behaviours.
