Nurse Practitioner Transformation Lead (Full-Time, Regular)

The full-time Nurse Practitioner Transformation Lead will play an integral role in advancing TVFHT’s commitment to increasing attachment to teams through Nurse Practitioner as Most Responsible Provider (NP‑MRP). This impactful transformation will improve service access through leading NPs to shift to a formal MRP role & support NPs and teams to achieve full scope. It will involve support of attachment work and developing metrics around complexity, panel distribution, NP‑MRP models and model outcomes. The role will also support future key service NP initiatives to offer impactful services to improve access and health outcomes of the communities we serve. It is expected that this role will guide clinical work and NP MRP transformation at the site level.

This position will serve London, Middlesex, Elgin and Oxford communities in alignment with Provincial scope and opportunities.
