Position Summary:
The Nurse Practitioner (NP), Reactivation Care Unit, works with the inter-professional team and physician partners in theAccess, Flow and Transitions Program,to meet the unique needs of our patients. With a focus on medical management and optimizing function and disposition planning, the NP works to full scope of practice within the Reactivation Unit to support the medical and psychosocial needs of the patients.
The NP will primarily work at the Humber Church site but may be required to support the Reactivation Care Unit at the main hospital site...
