Roles and Responsibilities

• Provides holistic assessment of and care for patients and families with a focus on health promotion, disease prevention, health maintenance and rehabilitation
• Completes health histories and physical examinations using assessment tools as available and applicable to assess strengths and needs
• Formulates and communicates diagnoses, determines the need for and orders the necessary diagnostic tests as per scope of practice, including labs, X‑Rays, ECGs and U/S and interprets the results
• Orders pharmacotherapy safely, according to most current guidelines when appropriate
• Develops and plans a plan of care that considers the individual patient's health status, preferences as well as current best practices
• Determines the need for and implements health promotion and disease prevention counselling, or refers to existing services in the community
