Nurse Practitioner

We are seeking a caring and compassionate Nurse Practitioner to provide primary health care within the Ontario Ministry of the Solicitor General’s correctional facilities. The role focuses on health promotion, chronic disease management, pain and symptom management, and mental health and addictions care.

Responsibilities

• Assess, diagnose and prescribe treatment with an emphasis on health promotion and disease prevention
• Communicate diagnosis and treatment plan options to patients, staff and external stakeholders
• Collaborate with an inter‑professional team to develop the patient plan of care and implement health promotion strategies
• Advocate for and facilitate self‑care principles, involving offenders in their own plan of care
• Act as a resource for patients, staff and external stakeholders
• Respond to emergency situations and implement appropriate interventions
