Key Responsibilities

• Provides comprehensive primary care to residents.
• Is knowledgeable of the Resident Bill of Rights and endeavors to respect and promote it.
• Conducts person‑centered health assessment/physical of residents on admission with specified time frame.
• Complies with applicable provincial legislation pertaining to long‑term care, Fixing Long‑Term Care Homes Act.
• Participates in regular focused resident and family care conferences.
• Manages the care of residents by providing pharmacological, complementary and/or counseling interventions.
• Provides outreach and transitional services to LTCH residents who return to the community.
• Collaborates and consults with physicians, director of nursing and personal care nursing staff, interdisciplinary team and external resources regarding resident plan of care.
