Summary The Mobile Wellness Case Manager utilizes advanced clinical knowledge to contribute to the efficiency of service delivery and advancement of practice and interventions on the Mobile Wellness Team. The Case Manager provides services at various sites in the community including private homes. Services offered include assessment, intervention, treatment, outreach and/or consultation, and referral for clients who have a Serious mental illness and their caregivers/families. The Case Manager understands the available community services and resources and demonstrates strong system navigation skills. Case consultation, case conferencing, and advocacy with community partners and service providers are integral components of this role. Working within an interdisciplinary team and guided by the Seven Grandfather Teachings, the Case Manager collaborates and consults with internal and external partners to facilitate referrals, coordinate care, and ensure service continuity.
