Shape lives positively as a Mobile Shelter Support Program Case Manager at The Neighbourhood Group (TNG). Provide essential support for adults facing homelessness and complex challenges across the Greater Toronto Area.
This regular full-time role focuses on direct case management, guiding clients in shelters, respites, and encampments towards their self-determined goals. You will address a range of issues from mental health to housing support, using a harm reduction and trauma-informed approach. Regular on-site visits will be essential in delivering comprehensive care.
Key Responsibilities:
• Conduct intake and assessment interviews with participants
• Coordinate referrals and advocacy for health, housing, and legal support
• Support clients in achieving self-directed goals
• Maintain community support through home and office visits
