Manager of Clinical Practice

Position Overview

One‑year contract position following a hybrid work model requiring onsite presence two days per week at our Kingston location. The contract is expected to begin in mid‑July. The Manager of Clinical Practice will lead our Bundled Care Program and support clients and caregivers through high‑quality, community‑based care.

Responsibilities

• Lead delivery of holistic care by collaborating with nursing, personal support workers, allied health professionals, and social workers.
• Partner with hospitals, Home and Community Care Support Services (HCCSS), and other community organizations to ensure safe and effective patient transitions.
• Provide mentorship, supervision, and performance management to personal support workers and nurses within the program.
