Responsibilities

LPNs are responsible for providing planned, patient‑oriented, goal‑directed nursing care in accordance with provincial standards of care. LPNs are an integral part of a multi‑disciplinary team who build trusted relationships with residents, families, and colleagues through continuous, ongoing communication. LPNs implement and evaluate care plans, making changes as required, and complete and ensure residents’ clinical assessments, care plans and documentation remain accurate and up to date. LPNs teach, delegate, and direct unregulated care providers, providing them with coaching and mentorship.
