Responsibilities

• Act as a Case Manager, accountable for assigned caseloads; assess client needs, determine service needs, negotiate service options, care planning, and coordinate transitions across care settings.
• Perform interventions and restricted activities for clients as required according to client need.
• Work as a mobile worker, travel up to 100 km/day caring for clients in their homes or in rural communities.
