Indigenous Discharge Planning Coordinator

Position Overview

Shared Health leads the planning and coordinates the integration of patient‑centered clinical and preventive health services across Manitoba. The organization also delivers some province‑wide health services and supports centralized administrative and business functions for Manitoba health organizations.

Experience

• Minimum of 2 years recent experience working with Indigenous patients in an acute care setting.
• Experience/knowledge of acute, primary and long‑term care systems & processes is desirable.
• Must have extensive experience working in and with the Indigenous community and be familiar with available resources both locally and in rural locations.
• Must have knowledge of the First Nation and Inuit Health Branch policies and procedures; functions and roles of Nursing Stations; First Nation Health Centres; Indigenous Services Canada, etc.; and related communications technology as it pertains to pati...
