Responsibilities
- Facilitate communication and collaboration among nurses/participants/staff for our drop‑in programs
- Establish and coordinate relationships with other healthcare services/external agencies for program development and implementation
- Coordinate care plans and services with external partners
- Research and identify potential resources and grants for health care
- Prepare and submit grant proposals to secure funding for healthcare programs
- Adhere to PHIA/FIPPA regulations and maintain strict confidentiality of participant information
- Facilitating Health Programs: Engaging and Presenting to Participants
- Organize and maintain participants’ medications, ensuring all prescriptions are current and accurately documented on MAR (Medication Administration Record) sheets. Dispose of any unused medications after two weeks, following program protocols ...
