Responsibilities
- Facilitate communication and collaboration among nurses, participants, and staff for our drop-in programs
- Establish and coordinate relationships with other healthcare services and external agencies for program development and implementation
- Coordinate care plans and services with external partners
- Research and identify potential resources and grants for health care
- Prepare and submit grant proposals to secure funding for healthcare programs
- Adhere to PHIA/FIPPA regulations and maintain strict confidentiality of participant information
