Salary Range: $60,000.00 To $62,000.00 Annually

The Opportunity

We’re looking for a Health Navigator. This role will coordinate the highest standard of care services provided by the care team and will assist in maintaining a safe and secure environment for residents, their families’ and other staff.

What You’ll Do

• Assess, plan, and evaluate individualized care for residents regularly.
• Coordinate resident assessments and service plans.
• Liaise with healthcare professionals and maintain accurate records.
• Respond to health emergencies and ensure compliance with safety policies.
• Coach Care Services team members.

What You’ll Bring

• RN or RPN designation with current registration.
• Knowledge of seniors’ retirement housing and relevant legislation.
• Strong communication and organizational skills.
• CPR & First Aid Certification.
