Key Responsibilities Clinical Care Deliver safe, effective, and evidence-based medical care to patients through face-to-face, telephone, and digital consultations.

Assess, diagnose, and manage acute and chronic illnesses in accordance with current NICE guidance and local clinical protocols.

Provide health promotion, screening, and preventative care to support improved patient outcomes.

Manage patients with complex, multi-morbidity needs, utilising clinical judgement and appropriate referral pathways.

Prescribe medications appropriately and cost-effectively, ensuring safe monitoring and review.

Patient Management Maintain accurate, contemporaneous, and legally compliant clinical records using electronic patient systems.

Provide continuity of care where appropriate, supporting holistic management of patients physical, psychological, and social needs.

Undertake home visits when clinically necessary.
