Executive (Primary Care Network)

The PCN Executive supports the implementation and performance of Primary Care Network (PCN) and Healthier SG (HSG) programmes. The role focuses on data monitoring, programme coordination, patient engagement initiatives, and quality improvement projects to drive programme outcomes, stakeholder engagement, and compliance with MOH/AIC requirements.

• Monitor programme performance and outcome indicators, and prepare reports and presentation materials for management updates.
• Drive, coordinate and implement initiatives to improve clinic participation and achieve PCN and HSG performance targets.
• Oversee patient engagement campaigns, including outreach efforts to encourage follow-up care and close care gaps.
• Lead and coordinate projects aimed at improving health outcomes, such as vaccination uptake and engagement of non-responsive HSG enrollees.
