Director, Integrated Care Solutions

Reporting to the Executive Director, Health System Integration & Partnerships, you will lead the charge in building and strengthening care initiatives that reshape how health care is delivered. You will champion partnership opportunities that create meaningful impact across our health system.

Annual salary is between $120,000 - $140,000 per year. Compensation is based on experience. Performance bonuses are also available.

Job Description

• Build and maintain strong relationships with hospitals, community organizations, and key stakeholders to identify and advance partnership opportunities aligned with organizational and health system priorities.
