Continuing Care Coordinator - Continuing Care

Overview

Nova Scotia Health is seeking a Care Coordinator in Continuing Care. The Care Coordinator is responsible for the assessment, care planning, authorization of services and ongoing case management of clients referred to Continuing Care. Care Coordinators help provide services and programs offered by Nova Scotia Health Continuing Care and the Department of Seniors and Long Term Care to assist individuals and families cope with acute or chronic health situations in the community. They identify goals, unmet needs, and resource requirements, and help formulate a plan to meet identified goals with clients and their care partners.

Responsibilities

• Assess clients and determine care needs and goals.
• Develop and authorize services and make referrals/linkages to other professionals and resources.
