Job Summary

ISTC is seeking an enthusiastic and autonomous Band 5 practitioner on a full‑time basis to provide intensive home‑based treatment for older people with acute or severe mental health problems. The role will involve working as part of a multidisciplinary team, delivering high‑quality care and supporting service users across Calderdale.

Responsibilities

• Provide home‑based treatment as an alternative to hospital admission where clinically viable.
• Reduce the length of inpatient stays and improve patient choice and experience.
• Work for short, intensive periods with people experiencing acute or severe mental health problems.
• Deliver planned care in the service user’s own home and access community resources.
• Support service development, manage enquiries and direct referrals, and participate in the 24‑hour duty rota.
