In this ongoing position, you will provide crucial primary health services, emphasizing health equity and education. Work within an interdisciplinary team to address the needs of individuals facing barriers to care while participating in on-call services and maintaining high standards of medical records and clinical practice.
Key Responsibilities:
• Conduct comprehensive health assessments and treatment plans
• Advocate for health promotion and disease prevention
• Share on-call responsibilities in a rotation
• Ensure accurate medical documentation is upheld
• Involve in clinical guideline implementation and case reviews
Requirements:
• Licensed Family Physician with a CCFP certification
