Community Care & Independence Coach

MINDS is seeking a Community Care Associate (CCA) to provide skills training and intervention for Persons with Intellectual Disabilities (PwIDs) across community settings in Singapore. The role emphasizes person-centered training, care plan development, and collaboration with social workers and healthcare professionals under CSS.

You will maintain client progress documentation, input into care plans, and work with caregivers to build support networks in the community.
