Caseload Support Worker Bissell Centre

Be a key player in Bissell Centre’s mission to eliminate poverty as a Caseload Support Worker. This role focuses on flexible, participant-centered case management in a collaborative environment.
As part of the Homeless to Homes team, the Caseload Support Worker will assist individuals and families in achieving housing stability. This position involves providing case management for those graduating from Intensive Case Management, as well as supporting participants exit rental subsidy programs and other transitional stages. Utilizing a strengths-based approach, your work will empower participants in need of housing stability and comprehensive support.
Key Responsibilities:
• Manage diverse caseloads for housing stabilization
• Conduct comprehensive participant assessments
• Triage participants for support level determination
• Monitor participant progress and recommend re-entry to ICM
• Develop individualized service plans promoting independence
