The Case Manager is a member of the multidisciplinary treatment team. Major functions include conducting psychosocial and CD assessments, gathering info from family and other providers, identifying discharge needs, and complete discharge planning. Our Case Managers also assist with placement and other discharge planning needs including scheduling appointments for aftercare.
